Claim Child Benefit

Important information

Before completing the claim form you must read the Child Benefit Claim Forms Notes (PDF 101K).
To be able to view and print out the claim form you must have Adobe Reader version 7.0.8 or later installed on your computer. Adobe reader is provided for download. The claim form will not work with versions earlier than 7.0.8.
To save the claim form to your computer, click on the link and press the save button in Adobe reader.
After completing the claim form you will need to post it to the Child Benefit Office, with the child’s original birth certificate or adoption certificate. The address is:
